Medication Use Management

Objectives/Activities

1. The resident will become familiar with various facets of Medication Use Management by completing the following activities:

a. Complete assigned didactic pre‐readings and be prepared for an interactive discussion on the material.
b. Explain the role of the formulary system and drug‐use evaluation in promoting rational drug therapy.
c. Describe the economic evaluation of new drugs and technologies.
d. Participate in active Medication Use Management work during rotation week.
e. Understand the importance of communication and the tools used.

2. The resident will become familiar with the role of the VIHA Pharmacy and Therapeutics (P&T) Committee by completing the following activities:
a. Describe the role and functions of the local, VIHA and Provincial P&T Committees.
b. Describe the membership of the local, VIHA, and Provincial P&T Committees.
c. Name the co‐chairpersons and the secretary of the VIHA and Provincial P&T Committees.
d. Attend one VIHA or Provincial P&T meeting.
e. Describe the role and function of the VIHA/Provincial P&T Subcommittees

3. The resident will become familiar with the role of the Health Authority Medical Advisory Committee (HAMAC) by completing the following activities:
a. Describe the role and functions of HAMAC.
b. By applying available intranet tools to research decisions of HAMAC

4. Understand the role of the Canadian Agency for Drugs and Technologies in Health (CADTH) plays in Hospital Formulary Management. Until April 2006, CADTH was known as the Canadian Coordinating Office for Health Technology Assessment (CCOHTA). By way of didactics and hands on application of Health Canada’s website, the resident should be familiar with the following functions of CADTH:
a. Health Technology Assessment (HTA) program and how applications are made.
b. The Common Drug Review (CDR)
c. The Canadian Expert Drug Advisory Committee (CEDAC)
d. The Canadian Optimal Medication Prescribing and Utilization Service (COMPUS)
e. The COMPUS Expert Review Committee (CERC)

Reflections


July 8, 2009
I have just finished three days of my Medication Use Management (MUM) Rotation. Instead of making me read many documents, my preceptors (Lara Campbell and Richard Cormier) have assigned me a project. I am helping them re-organize them replace some of the therapeutic substitutions that will be removed at VIHA with more user-friend charts. These charts will be used not as therapeutic substitution but as a “guide” so pharmacists can use their professional judgment to dispense equivalent products. So far,  I have made charts for calcium salts, iron salts and hemorrhoidal products. I will try to post a chart if I am allowed to!
Next week, we will have didactic sessions on topics such as formulary, P & T committees, and databases such as CDR ( Common Drug Review).

Today, I attended didactic sessions as the final requirement for the Medication Use Management rotation. Although the material could have been really “dry”, Richard and Lara managed to make the session interactive and more interesting. We talked about different topics such as restrictions, drug shortages and formulary changes and applied real-life examples. A few issues that we discussed were the removal of nystatin powder of the market and formulary change from tinzaparin to dalteparin. We also looked at the CADTH website and looked through the Common Drug Review (CDR) database. A few things I learned about from the session and the rotation that I did not know before were:
  • The P & T Committee makes recommendations (to add or not to add a drug to formulary), however HAMAC has the ultimate power to approve it.
  • The Antibiotic Review Subcommittee sometimes makes recommendations to the P & T Committee.
  • The P & T Committee does not just deal involve the formulary. Other issues they are responsible for are medication safety, adverse drug event monitoring, medication error prevention, etc.
